Call dentist: This can happen.  Take the pain medication perscribed by the dentist or oral surgeon.  If they did not prescribe anything you can take ibuprophen 600 mg and 2 tylenol (acetaminophen) every 6 hours, of these are safe for you.  Call  your dentist or oral surgeon about your severe pain.

See below: Contact your oral surgeon or general dentist about any concerns. What are the variables, were the teeth impacted?  If so, healing may take longer and be more protracted.

Headache: Do you normally get headaches?   if you do, then I would not be overly concerned.    If you do not normally get headaches, then I am somewhat concerned, and think you should tell the doctor who did the procedure.  I imagine they gave you some pain meds.  You could take a dose and see if that helps - along with some extra hydration.

Seek help: Any sudden pain, bleeding, or (as in your case) swelling should be evaluated immediately.  See your Oral Surgeon, General Dentist, General Physician, ENT Specialist, Urgent Care, or ER.  Seek immediate care.

7 days post wisdom tooth extraction & scab came off from lower. rinsed mouth with salt water and noticed a brown object in the sink.
there is a hole on bottom. no severe pain. top has scab intact. i do have tmj.
is this ok? is dry socket possible?

Yes can: The blood clot in extraction socket helps regenerate bone .If the lower right socket is empty &amp; starts smelling and pain gradually increases ,you must see the Dentist who did extraction , he might have given you Antibiotic as prophylaxis otherwise if needed he will prescribe Antibiotic and pain medication as needed .
TMJ problem at times increases after extraction due to wide opening of mouth .

Dont dislodge clot: Do not rinse for the first 24 hours, after that rinse gently and never dislogde the blood clot. 
Follow the instructions of your dentist to the letter to avoid "dry socket".

No smoking: Do not smoke after wisdom tooth extraction.  You increase the chance of very painful dry socket.  Follow the directions from the dentist or oral surgeon that did the extraction.    Eat a soft diet, drinking is ok but avoid alcohol while taking pain medications.

Fluids encouraged: Fluids are encouraged after extractions, just from a cup or spoon, no straws.  The suction or negative pressure in the mouth can make the sites bleed.  I recommend clear then full liquids the first day, then soft foods for 4 to 7 days.  Avoid alcohol after anesthesia and if taking pain medicines.
