Calcium parathyroid: This is usually diagnosed in people with kidney disease.  If the calcium is low, or the phosphorus too high, or the level of Vit D too low, the parathyroid makes too much PTH, which causes bone disease. Treatment targets the cause:  vit D (calcitriol), phosphate binders, special diets.

Vit D analogue: Not regular vitamin d.  You need calcitiriol, zemplar, or Hectorol which are already partially activated and don't require the kidney's final hydroxylation step to make them work.

Depends: It depends on the original level and what it's now. You do need to monitor the level as it's a fat soluble vitamin and can build up. That being safe, 2000 iu of vitamin d3 is quite safe.

Fracture: Secondary hyperparathyroid refers to increased parathyroid due to low calcium commonest cause is low vitamin d as such if this has been going for long like nutritional osteomalacia gluten enteropathy renal problems they can be associated with incresed fragility.

Yes: This would generally be in the case of severe or very low vitamin d over a period of time.  In such a case, the calcium would drop and increase the work of the parathyroid gland.

: Hypocalcemia (low calcium level) resulting from severe vitamin D deficiency induces overproduction of parathyroid hormone, which is a secondary hyperparathyroidism.  No tertiary hyperparathyroidism.

Both primary and sec: patient likely has combination of primary and secondary hyperparathyroidism.calcium level is high 10.6, intact PTH is not suppressed.  This would indicate possible underlying primary hyperparathyroidism.  While vitamin D deficiency can cause secondary hyperparathyroidism, usually calcium levels are not elevated in this context. recommend repeat evaluation after vitamin D replacement
