Unknown: Severe leg pain can be caused by many things.  Think about what's in there-blood vessels (disease can cause pain), muscles (rarely if ever a primary cause of pain), tendons and ligaments (can be torn in accident), joints (hip and knee can both cause pain), and nerves.  In addition, damage to various structures in the back can cause leg pain.  To find out, see doctor and get mri.

Sciatica: 95% of people experience an episode of back pain and leg pain in their lifetime. Most episodes only last a few days but may require bedrest doing that time. Fortunately he has medication. Since he has already seen a doctor, he should wait a few days and return to his doctor if symptoms persist.

No: No, many structures from the low back can produce pain in the leg. This includes the discs, the facet joints as well the covering of the nerve sac called the dura. In fact i see more pain from some of these other structures than true sciatica. The quality of the pain from sciatica is also different.

Sciatica: is an extremely painful condition and he should call his prescribing doctor to rule out the possibility of a herniated disc. He will need anti-inflammatory and pain medication.

Sometimes: If it is switching sides, more than one nerve is involved. Sometimes a very big central disc herniation can cause this. Consider getting an MRI if this is getting worse or not improving.

Should be evaluated: If back and leg pain persist despite exercise and rest,  it likely reflects a more significant problem causing nerve compression and should be more thoroughly evaluated.

I would see-: -a fellowship trained spine surgeon to be evaluated . Not all need surgery, but a good place 2 start. May need referrals 2 a pain clinic or neurologist.

i have sciatica lower back and leg pain periodically.  i regularly perform the key hole stretch, and strengthen my core and back with regular planks, jump rope and yoga stretches.  i periodically have therapeutic massage as well to relieve the pain.

Next steps: Sciatica that lasts longer than 6 weeks or is associated with progressive  symptoms needs workup. Lumbar xrays will assess for abnormal movement in your bones. Lumbar MRI will assess for disc bulges impacting the nerve and assess the spinal canal diameter.  If your symptoms don't respond to activity limitations, physical therapy or lumbar steroid injections, then surgical options needed.
