i have been in a cast for 6 weeks for fractured scaphoid i fell and am in a lot of pain my cast comes off next week  will i end up in another cast?

Possibly: Scaphoid fractures are casted for longer periods of time because they don't heal as well as other bones.

is it okay to sleep with a fractured scaphoid in a cast with your hand upside down?

Yes: Best to keep the hand and wrist elevated to reduce swelling. Position while asleep will not effect healing of the fracture as long as you have a well fitting cast

6 weeks: Fractures take 6 weeks  to heal, but the pain should start getting better within the first 10 days.

what is the length of the cast you get for your scaphoid?

Varies: The scaphoid is a slow bone to heal and orthopaedic surgeons having different opinions on what type of cast and for how long to cast.  Some will cast above your elbow, and all the way to the tip of your thumb.  Others, below the elbow and just below the joint in your thumb.  A cast may be required for a minimum of 6 weeks, and possibly as long as 3 or more months.

?: With this hx one should consider an improperly  positioned cast. If u were my patient i would have addressed this right away. U should b comfortable right away in a cast.

Chances are good. : But I would listen to the doctor who is in charge of your treatment. It takes bones approx 6 weeks to heal give or take two week in either direction...

Digital nerves: Could well be secondary to injury to digital nerves, and less likely a more proximal median nerve problem.  Typically in a child, recovery of nerve fnctn is rapid, perhaps within 4-6 weeks.  Let your doctor know of the problem, but these issues are usually short-term with good outcomes.

Therapy: Therapy is probably all that is needed for just thumb numbness. If any other fingers are numb he should be checked for carpal tunnel syndrome.

Depends on location: If the scaphoid is broken in the middle of the bone or closer to the forearm, healing is more difficult. These areas of the scaphoid do not have a very good blood supply. Yes: Scaphoid fractures are sometimes difficult to visualize on x-rays, especially initially after an injury. If you have tenderness at the base of your thumb near the wrist, it is reasonable to splint or cast the wrist until follow-up x-rays confirm the absence of any fracture. Occasionally, an MRI study can be useful to rule out a scaphoid fracture definitively.
