Therapy: Therapy is probably all that is needed for just thumb numbness. If any other fingers are numb he should be checked for carpal tunnel syndrome.

Depends: Numbness after cast removal is not uncommon, but should improve within several days now that the compression from the cast is removed. Pain may be from either stiffness associated with the immobilization, or perhaps from the fracture site itself. If that doesn't improve within the week then he needs to be reevaluated to differentiate between these two options.
