HIV Is Treatable: Hiv is a permanent, life threatening disease that has no cure.  There are very good and effective medications that keep the HIV suppressed and from harming the human.  There are very good medications that keep the baby safe during pregnancy and after.  I would advise you to make sure your friend is seeing an HIV expert as part of her health care team.

Yes: Every pregnancy and every body is different.  Especially in the first half of pregnancy, you would expect that the enlarging uterus will be "low" on your abdomen.  As the pregnancy progresses the fundus (top of uterus) becomes palpable higher above the belly button.  This is actually one of the things your doctor will measure when you go in for exams to make sure the baby is growing as expected

Possibly: There is no doubt that as u age and have more pregnancies it is more and more difficult to maintain muscle definition.  This in turn could lead to carrying lower, on a good note labor tends to progress much quicker.

Good : If you have had 2 normal pregnancies before, your chances are good. you are at high risk early, so as soon as you think you are pregnant, you need to get in and get checked.

while in passenger side of parked car w/window down, lady carrying coughing baby walked right past me. it was quick but coughed right as she walked by my open window facing me. sounded like baby had bad cough. i'm (sadly) not vaccinated. risk level?

: The risk of you getting COVID from that exposure would be low-even if we assume the baby had COVID (which is likely not the case as there are many other reasons for a baby to be coughing). Very little of the virus in the droplets from the cough would get to you from an open car window making the risk of getting the virus low. Get vaccinated to make your risk even smaller!

Unlikely: I'd ask your OB for a better answer, since s/he would know your case the best.For most healthy women, carrying a toddler will not trigger  a miscarriage.

Twin preg high risk: The uterus is really designed to only carry one fetus, however generally twins whose mother is otherwise healthy do well.

Let OB know: Today's technology is good at handling pregnancy induced hypertension, but clearly having eclampsia during your first pregnancy puts you at huge risk for the second child. Letting your OB know, making all appointments, following advice closely, and letting the OB know of any fluctuations in your condition will help the most. Also complying with any special instructions, and taking meds correctly.
