Many people do: Sacroiliac joint is the most complex joint in the spine. It moves in six planes of motion and is l shaped. Has innervation from the back and in the front. It can become misaligned and cause a lot of pain due to abnormal friction of ilium on sacrum. In someone young like you it is usually due to pelvic imbalance from short leg syndrome or psoas muscle spasm.

Not typically: Sacroiliac dysfunction is caused by too little or too much movement at the sacroiliac joint. It is at the meeting of the iliac bone (you can feel it when you rest your hands on hips) and lower spine. Muscle imbalance of the gluteal and core muscles most common cause.  In families with hyper mobility or inflammatory arthropathies, it may be more common.  Pt is the 1st step in treatment.

Sacroiliac pain: Sacroiliac dysfunction : where your spine end , that is sacroiliac area, s1-2-3.....The joint abnormalities produce pain lower back similar to sciatica, pinch nerve, radiculopathy, or disc hernia. That is related with abnormal motion and fixation, and the diagnosis is at examination and mri.

Cold, PT, NSAIDS: There are many treatments, starting eith cold compresses, and nsaids (like ibuprofen); there are also topical nsaids (like voltaren (diclofenac) gel.) physical therapy sometimes helps, and in severe cases, an ultrasound guided injection into the area is done.

Depends: Si joint dysfunction can be chronic or acute, depending on the cause and length of time one has had symptoms.  In general, si joint dysfunction is due to inflammation of the si joint; prolonged inflammation can result in arthritic changes to the joint surface.  Inflammation can be reversed (thus tends to be acute) but arthritic changes can not be reversed. See your doctor if in prolonged pain.

Yes: Si joint fusion is a option if you have exhausted all conservative measures including 6 months of treatment.

Fuse: That would be an option but would encourage you to discuss this with your physician to see it there are any other medications that can be taken for chronic back pains. Surgery is never the final solution as there are cases where the condition actually gets worst with a procedure.

Yes: It is important to have a thorough physical therapy evaluation to determine what is impacting your sacroiliac joint dysfunction.  Treatments should focus on manual tissue and joint techniques to align and mobilize the area, as well as stabilization exercises to maintain equal forces around the joint and to protect the joint.

Not necessarily: These are general diagnoses that have little description as to the pathology that is occurring. An x-ray can occasionally identify pubic fragmentation in chronic osteitis pubis or sclerosis in si arthritis, but their absence doesn't mean that it's not there. An MRI is more sensitive and is good at picking up acute inflammation, but relatively poor in chronic situations of inflammation.
