The : The cleaning that you had is a superficial removal of calculus and stains. I would find it highly unlikely that the cleaning necessitated the need for two root canals.

Was the whitening procedure in office with a laser/bright light or the take home tray type? Also, are the two teeth in question in the front of your mouth or in the back, and do they have previous large restorations?

There are so many variables here, it is hard to answer your question definitively without knowing all the particulars. The bleaching gel and hot light have been known to cause sensitivity in whitened teeth, which is usually transient. What does your treating dentist feel is the cause of the nerves dying and the need for the two root canals?

What got cut?: Hard to tell from your question, maybe your cheek or gums were inadvertently cut during the root canal procedure?  Warm salt water rinse 2x per day may help.  One way to prevent such a problem is to do the root canal with a rubber dam in place, which holds the tongue and cheek out of the way and protects you (and from other harms). If this wasn't used, find a new dentist.  Period.

Needs re-evaluation: See your treating dentist. Root tip may be infected or the tooth may have a fracture. Needs to be tested and visualized to know for sure.

Fracture?: The tooth may have a fracture that is very difficult to diagnose in some cases. If the tooth is fractured, extraction with an implant is a possibility.  A three unit bridge or a partial denture are also possibilities. Best to discuss these options with your dentist.

Agree : Agree that what you are asking is a legal quesition not a medical question.

I would contact your state dental association (not dental board) and inquire if they have a mediation process.  This is usually a better route to take since many lawyers will not take a case like this because the potential award in a claim like is this is too small for them to make enough to justify their time and efforts.  You will have to bear the burden of proof that the dentist's actions were negligent and deviated from the accepted standards.  As dr. Scharf stated, it is not uncommon of a file to break off in a tooth during a root canal procedure.

Personally, i would not charge for a procedure that could not be completed satisfactorily as a gesture of good will.

Abcess: The lesion should resolve once the treatment is completed.  It takes anywhere from 3-6 months for the bone to heal in.  Sometimes, if the infection has been present long enough to create scar tissue, a second procedure may be necessary.

Dentist can re-check: A person who started a root canal but did not finish, may or may not have a temporary crown on the tooth. The tooth may be more likely to crack or break if it is not properly crowned. It may be more likely to get decay if it is not protected and sealed off from the bacteria and food substances. It is best to check with a dentist to get the tooth well-protected.

Have faith in your dentist. He/she is only trying to treat your problems and 
prevent you from loosing your teeth.

Local Dental Society: You can give your local dental society a call.. They may be able to steer you in the right direction for programs that you may qualify for. A local dental school or hospital with a dental residency program should be able to treat you at a reduced fee. There's always carecredit if you wish to pay over time.

No: Proper treatment includes "pain control".  Today's dental community take this very seriously and us local anesthesia to control the comfort of the patient.  Call your dentist to discus their approach and find the person that make you feel comfortable with your decision..
