Needs exam: This could be many things. Some illnesses produce a rash on feet, hands or around mouth &amp; bottom. Some kids walk on stuff their skin reacts to. Bug bites can trigger spots. It will need to be evaluated by direct exam to sort it out.

Urgent care: If your sure the fever hasnt broken, then a visit to urgent care is warranted.I f the fever has broken he has a harmless virus known as roseola.

Yes/no: If concerned, you can take a diaper with the material to your pcp and a test can be run on the debris. Watermelon can pass through the kids system with little alteration of the color as can things like tropical punch (in the red of course).

Hard to tell: Without seeing him it is difficult to tell.  If it is not gone in a few days i would suggest seeing a dentist or physician for a diagnosis and treatment.  It could be a virus, or something else..

Rashes: Small red dots any where on the body could be petechiae and should be evaluated by a doctor.   Rashes are impossible to diagnose without seeing them.  Petechiae are small (1-2mm) red or purple spots on the body, caused by a minor hemorrhage (broken capillary blood vessels).  See attached picture.   Unlike most other red rashes they do not go away with pressure.  Petechiae always require evaluation.

Common finding: Many viral and some bacterial illnesses will have this finding.Your doc can sort out the possibilities and decide if testing or treatment is needed.Sometimes a call to the office and discussion with the staff can help you decide if you need to come in.

Could be cellulitis: Typically cellulitis is tender and warm to touch.  It may also be associated with swelling but not necessarily.  Given your history, you are at higher risk for cellulitis.  You should probably be evaluated by a physician in person, particularly if you have diabetes, note spreading redness, or develop a fever greater than 100.4F.

Doctor can examine: Red spots are hard to describe or diagnose in words, and can be from many causes such as:  viral infections, other infections, medication causes, strep throat, hot-tub rashes, heat rashes, reactions to creams or lotions, allergic reactions to various things (including foods), internal disorders, etc...  That means a rash that persists or is worrisome in appearance needs to be checked by a doctor.
