See doctor: I don't know what it is so would hae to see it.  is it acne, pyoderma, erysipalus, pyoderma, urticaria.  Gosh you have to see it to have an opinion.  is it new or congenital?  infections on the face can be dangerous and should be seen rapidly so they can be cured befor you need to be put in a hospital.

Hard to know: It would be easier to respond if you described the red area in detail, along with exact location. This could be a simple "storks kiss" birthmark common to the nape of the neck or something else. I do not think it worth a special trip to the doc, but bring it up at your next visit. If it is new or troubles you  contact her doc.

Many things: Many things can cause the symptoms you describe including viral infections-enteroviruses, measles, german measles etc., bacterial infections with toxin production due to staphylococcus and streptococcus, rocky mountain spotted fever and other rickettsial infections, syphillis as well as drug reactions.  You need to be checked out by a doctor.

Probable keratosis: Most nevi that transform to the malignant state do so by enlarging , usually becoming darker in pigmentation, and potentially ulcerating.  For a freckle to lighten in color and become swollen can not by definition transform to a red spot.
If the lesion enlarges, darkens and shows signs of peripheral inflammatory changes, bx should then be performed.
