Rash: Hard to tell especially that it is one side, can be seborrheic dermatitis, contact dermatitis, etc. You need to have it examined.

Call - us  Wart.: Raised roughened areas on the side of the big toe, especially at your age  are restricted to only a few conditions. A wart will generally hurt when squeezed. A callus will generally hurt when pressure is applied by pushing on it. The problem is, to tell them apart, a good visualization of the bump  is needed. Best to see a foot specialist to prevent the lump from potentially spreading.

i lost my fingernail 3 weeks ago, when can i swim again? and how long should i keep wrapping it and applying disinfectant ointment? my fingernail bed now has rough skin on top and it's not so sensitive anymore. there's been no oozing, redness, etc.?

: These questions are best answered by your doctor after seeing your finger. What type of water are you swimming in (pool? Ocean? etc.)?
If your doctor sees that there is a layer of skin covering the area, this may be enough to stop the ointment and wrapping. You may need to cover it for swimming. Your doctor can tell by examining it what the next steps would be! Good luck to you!

Possible infection : Breast pain/redness is a sign of infection (cellulitis/abcess/mastitis). You should get seen for proper treatment.  Another concern:  'inflammatory breast cancer' can present as redness in skin so this should also be considered and ruled out.  Hope you are better soon!

See your doctor, get a precise diagnosis & treatment.

Sunburn: Sunburn can give those dark spots on skin exoosed to sunlight.  Use sunscreen and avoid long hours exposure to sun.
