Ischenic colitis: Heals in 3 ways. 1.the colon dies and perforaates. 2.heals completely and is back to normal, 3, Scars down and becomes stenotic,

Not necessarily: There are many reasons for enlarged lymph nodes;  most common is reaction to infection/inflammation which maybe your case but they can also enlarge in reaction to cancer nearby and in cases of lymphoma. These possibilities shluld be sorted out after colonoscopy and biopsy of any colon lesions

: You can have disease that is not apparent to the naked eye. It may be microscopic in nature and too early to cause a change in your blood work.

Hematochezia: It sounds like ther are many important variables regarding your question that make it a complex one.  Yes xarelto can thin your blood and make you more prone to bleeding.  Opioid medications usually have the side effect of causing constipation.

Ulcerative colitis: No simple answer to care and treatment of uc. Start with a low fat, low gluten diet and a bowel regimen. Avoid alcohol and nicotine. Your primary doc will refer you to a GI doc for medications and colonoscopy. There are several medications that can help. Hopefully you will stabilize at this point. If not, surgical consult and procedures may become necessary.

Unlikely: Unlikely. But at least you should get a CBC done.

Probably not: Obvious gross bleeding would delay things until cause of bleeding is found. Minor bleeding from edge of stoma or stoma itself should not be of concern, nor should test for "occult blood" which is usually positive in colostomy patients.
