Hard to speculate: An exam to determine the type of rash would determine whether or not any or what would be needed. This could be an allergic reaction to some agent applied to the scalp, ringworm or something different. Treatments for one issue could aggravate another.Without a better description of the rash or an exam little can be offered.

Inf or tumor: Scalp rash due to infection rarely spreads to cervical lymph nodes.  One has to think that it is more likely neoplasm and of those it is the squamous Ca of the scalp that can cause a non nealing rash and spread to regional cervical lymph nodes.  Bx of course is indicated with initiation of proper treatment.

Unrelated: Scalp rash and joint disease are not symptoms of HIV. Both symptoms may or may not be related. Some connective tissue disorders like lupus may be manifested with both.  Obviously,  you need to be examined by a physician who would also order the appropriate tests
