does losartan 50 last 24 hours or do i need it twice daily ? i am rapid metabolizer?

: I was excited when losartan, the first ARB for treatment of hypertension was released. It was especially useful for patients that had side effects from ACE inhibitors. However,I found that in a number of patients,I had to prescribe it twice a day (every 12 hours) in order to attain good blood pressure control as opposed to later released ARBs.

if someone is a rapid metabolizer. does that affect eliquis ? could the eliquis not be working as well and not lasting since its metabolized quickly or does eliquis not get affected by the liver and metabolizing ?

: Since we don't have a commercially available blood test to measure its efficacy, you must look up a list of what supplements &amp; other medications to avoid or use with extreme caution. The good news is that you don't have to get regular blood tests to monitor Eliquis. It acts within hours&amp; lasts about 24 hours. is eliquis 5 mg twice daily still effective for me if i am an untra rapid metabolizer or could i require it taken 3 times ? is it possible it isnt reaching peak anti coagulant affect due to being metabolized so quickly ?

: There are a lot of hypothetical questions and reasonings here. Best discuss it with the prescriber. Never change the dose of a prescribed medicine without consulting the prescriber. Good luck

i take eliquis 2.5 mg twice daily for a clotting disorder. i am an ultra rapid metabolizer confirmed by testing. should i be taking eliquis three times daily instead incase i am metabolizing it too quickly ?

: Eliquis is typically dosed twice a day with the most common dose being 5 or 2.5 mg twice a day depending on the condition being treated or prophylaxed and specific patient circumstances ( kidney function/ age, etc). Your current prescriber would be best to make formal recommendations as to adjusting doses.

if genetic testing confirms you are an ultra rapid metabolizer (say of 2d6), what antidepressant choices do you have, if any?  or are you out of luck?

There are choices: There are antidepressants that are not metabolized by 2D6.  Zoloft, Celexa, Lexapro, (escitalopram) Luvox are a few.
However, knowing that you are an ultrarapid metabolizer does not mean that you may not benefit from medications metabolized by 2D6, it just means that you will require higher than average doses.

i've taken effexor (venlafaxine) for 1yr, but get brain zaps every day.  could be i'm a ultra-rapid metabolizer, sure.  but any chance it's a stomach absorp. issue?

Yes/NO: people who get interdose withdrawal symptoms from antidepressants that usually are taken once/24 hrs, those people usually are rapid metaboizers or ultra. we split the dosage over two or three doses per day. possibility of malabsorbtion is less likely

if you are a rapid metabolizer and you get interdose withdrawal daily, is pristiq (desvenlafaxine) the ideal antidepressant, since it bypasses the liver?

Accepted Answer

Not necessarily: About 45% of Pristiq (desvenlafaxine) is excreted unmetabolized by 72 hours post dose.  The rest is metabolized by CYP3A4 which is a liver enzyme.   It does avoid CYP2D6 which is the most common enzyme which metabolizes psychiatric medications.   I assume you mean that you are a rapid metabolizer of the 2D6 enzyme.
In that case, Pristiq (desvenlafaxine) may be a reasonable choice.

Quite the opposite: That non-alchoholic fatty liver indicates you have other problems and need to be careful of what you consume. I recommend you consult one of the companies that evaluates your genetics to determine your specific risks to best evaluate your treatment alternatives.

Yes: CYP3A4 is the main metabolic pathway. There is a lesser contribution from other Cytochrome systems as well
