27 y/o with hiatal hernia; was only prescribed ranitidine. what other treatments are there  & will i ever need surgery?  discomfort continues. thanks!

Maybe: Hiatal hernia and reflux ate two separate things. Reflux causes heartburn and regurgitation, and may be aggravated by a hiatal hernia. There are procedures to stop reflux, some of which include hernia repair. A large hiatal hernia may cause difficulty eating, early satiety, and intermittent chest pain among others. Surgery is the only way to fix them. Meds only control some reflux symptoms.

Need a consult: This constellation of symptoms is too complex to answer with a simple one or two symptoms. It will take a dialogue (questions and answers) with a gastroenterologist to get to a differential of possibilities and tests to reach a diagnosis.

Yes: hiatal hernia is a common mimicker for heart disease. However heart disease is the most dangerous etiology  for chest pain so need to have that checked out first

will otc nexium24hr help acid reflux caused by hiatal hernia? i currently take 300 mg of zantac qd and it helps very little, if at all.

The two are very: different drugs.. Zantac an H-2 blocker blocks the acid at the "target" and is effective for some people NEXIUM stops the acid from being released altogether...the latter is usually more effective in relieving reflux but takes longer..Get on it and be patient..about a week should result in improvement in almost 100% people!

No: If it controls your symptoms it is appropriate therapy. I assume there is planned follow up with endoscopy to monitor your progress.

Maybe just heartburn: ...But atypical chest pain especially in middle aged women is nothing to ignore. The potential origins may include: heart, lungs, mediastinum, esophagus (due to dysmotility disorders, gastroparesis, biliary/pancreatic trouble, and/or non-acid reflux), musculoskeletal, pericardium (sac around the heart), pleuritis, bronchiitis, or referred pain from a remote site. Further evaluation is warranted.

i have hiatus hernia. have been taking 40mg omprazole but recently have been vomiting occasionally. what is best time of day to take omperazole? should i take 20 before breakfast/ 20 before dinner? can i supplement with ranitidine before bed?

Yes: Probably the best time to take omeprazole is 30 minutes before the meal .. stop all carbonated beverages and never drink coffee or tea on an empty stomach .Try to  maintain hYdration.. also ,  when was your last upper endoscopy.   Yes , you can  take Zantac or it's generic before bedtime..

i have gerd and take omeprazole and zantac (ranitidine) for more than 2 years i've food stuck and recently diagnosed with hiatal hernia what should i do?

Concerned: I would recommend you follow up with a gastroenterologist . If your symptoms have persisted for this long with the food getting stuck, you may an an egd ( camera down esophagus) to look for a blockage or stricture.

Hiatal hernia: Most commonly patients with hiatal hernias will experience symptoms of reflux such as heartburn and regurgitation if they have any symptoms at all.  The vast majority of hiatal hernias  encountered are small asymptomatic hernias that require no intervention.  Only those hernias that are symptomatic should be treated.  If surgery is required, it usually can be done laparoscopically.
