Separately, a recording of your heart rhythm (24 hr, 48 hr, 30 days or longer) can help correlate your symptom of "painful protruding heartbeat" with your actual rhythm. From this, treatment, if necessary, can be determined.

Get it checked out: These are probably being palpitations that are commonly related to anxiety . You should see your doctor and have it checked out though (will need at least and EKG plus some blood tests).

Normal: 'hard' heartbeats usually means that particular cycle was either pumping more or less than usual amount of blood. If its not exercise associated, and you are generally healthy, it usually means too much caffeine, or too little rest.  Try less of one, more of the other. If it continues, consult your PCP.

i always feel rapid heartbeat when laying down to sleep. is this normal?

Yes: I see this often in patients, its unclear what is going on, but i think it may sometimes be related to the fact that when one lays down, more blood flow reaches the heart from the feet and causes the heart palpitations.  A solution may be to first raise legs in a chair for 15 minutes then lay flat. Would be curious to know if this helps.  Of course, one should rule out heart disease or svt ct.

Left side?: So, when you lie on your left side your heart is much closer to the inside of your chest wall and pretty near the stomach.  You might be much more aware of it beating in this position.  You don't say how old you are.  If you have high blood pressure or are over 60, and this is definitely new, you should just have your abdomen checked to be sure it is not an enlargement of heart or aorta.

It might be.: You are describing classic orthostatic changes that often cause lightheadedness or fainting. I would see your doctor of a complete physical examination.

Monitor needed: I feel that it's important to document what your heart is doing during these symptoms and if an EKG isn't helpful, then a heart monitor can be put on you as an outpatient and can be worn for days until it records your heart rhythm during your symptoms. Discuss this with your physician who can order the monitor. If needed other tests can look for heart or thyroid disorders or reactive hypoglycemia.

my heart have 1 or 2 very strong heartbeats after every few mins, but i notice them more when i am laying down or sitting. is everything ok?

Heart: Yes.  More fluid in your head and neck when you lye down and this makes heart tone more audible
