Your best bet is to consult a specialist in internal medicine who'll take a complete history&amp;system review, perform a thorough physical examination, order appropriate tests and then advise &amp;treat you or refer you to appropriate specialists such as a dermatologist or neurologist.

i have a knot in the lower right side of my back that i've had awhile, it starts to hurt extremely bad if i put pressure on it like when bending down?

Muscular v disc: This is either muscular issue if only several weeks. If longer, it may be from a disc issue that could be a tear in the disc or a herniation.  If muscular, it will get better by about 6-8 weeks and 90% of discs improve over time. Otc meds can help as can heat &/or ice as well as guided exercise & activity modification.  Lifestyle changes like weight control, not smoking & regular exercise are key.

Posture: Likely fatigue of muscles from posture and some muscle weakness. Do extra exercises for those areas and watch posture.

: This could come from a variety of situations, such as bruised tissues (muscles and ligaments) or something structurally wrong that is a bit deeper, like the joints (called facet joints or sacroiliac joints), or the vertebral bones.  A disc problem or spine subluxation can also cause tenderness to pressure of the lower back.  A through history and examination with your physician is warranted!

When bilateral vertebra can slip forward.Disc disease can also occur in young individuals usually symptoms more on one side than  other, sometimes into lower extremity.Sometimes imaging with x-rays and bone scan necessary.

Back pain: Back pain is one of the most common reasons for a visit to the doctor.  It's hard to assess what you may have without an extensive history and physical.  Start with some otc Acetaminophen or ibuprofen.  Use an ice pack or heating pad.  Decrease the volume and intensity of your activities.  If your pain continues and becomes severe, then ensure you have follow up with a doctor.

Muscle tension progresses through the day and is often worse in the afternoon or evening. The alignment of the neck may be incorrect due to a muscle strain, ligament injury, or early arthritis. See a chiropractor or neurosurgeon for evaluation and treatment.

Musculoskeletal: If you can affect this discomfort by bending, turning, or twisting it would point to a musculoskeletal problem and resting the area from strain should help. If the pressure is higher up other factors like kidney or gall bladder problems could be present and would warrant an exam.
