Pruritic skin rashes: Based on your age and description, the most common skin rash that are red, itch, and scaly is atopic dermatitis. Psoriasis could be similar but it is not as itchy. Either case, you can try to apply otc Hydrocortisone cream to the lesions a few times a day plus moisturizer generously to always keep your skin moist. If it does not improve or not completely resolve, you would need to see a doctor.

there are some particular moisturizers that you might try:
cerave sa
carmol 20
amlactin
all are over the counter and can be quite helpful.  Another good option for the shins if you can stand it, is to slather them with vaseline, cover up with tall socks, and sleep that way.  Sometimes makes a dramatic difference overnight.

Hypopigmentation: Postinflammatory hypopigmentation is a common cause of acquired hypopigmentary disorders. It can be a result of cutaneous inflammation, injury or dermatological treatment. Eczema can cause this condition.. Most cases of postinflammatory hypopigmentation improve spontaneously within weeks or months. Treatment can be with Elidel (pimecrolimus) cream, non-steroid.

Athlete's feet: Is caused by a fungus and requires moisture and warmth to grow. Shoes are the main culprit. Good hygiene and synthetic socks and wearing sandals will help. There are both topical and oral medications for treating this, depending on the severity. Discuss these with your doctor. Nail fungus is best treated orally. Lamisil (terbinafine) is a drug used most often. Laser plus topical anti fungal cream may also work.

Collagen: You have a knee scab that continues to be a problem years after the injury.  The problem is you no longer have normal skin there, you have collagen.  Try apllying a skin moisturizer every day for 1 month. This should help.  You can try other creams if no improvements after the month.
