Discuss w doc: One expects a physiologically appropriate increase in heart rate with exercise.  This could be magnified if your volume state is low (intravascular dehydration) .  Symptoms of dehydration coupled with the normal sympathetic stress of activity could explain this--but if you are still symptomatic after assuring adequate hydration you should see a cardiologist for monitoring/echo-stress study.

There : There is a possibility that ultram - also known as tramadol, will cause the allergy as well. I would like to refer you to an excellent article in the journal orthopedics (2007, vol. 30, p. 920) titled "drug selection in a patient with a history of hypersensitivity reactions: a practical approach for the orthopedist" by lisa a. Thompson and others. This is what the article says about tramadol: "tramadol is a centrally acting analgesic used as an alternative to nsaids or more potent opioids for the treatment of moderate pain. True tramadol allergies are rare (occurring in less than 0.1% in one study), and typically involve hives or pruritus, but can include anaphylaxis or stevens-johnson syndrome. Tramadol is structurally similar to morphine and codeine, so cross-reactivity can be expected. The package insert cautions against use of tramadol in any reported opioid allergy.â€ as you can see, the authors caution about use of tramadol when there is a history of allergic reactions to an opioid, which would include codeine.
 Hope that helps. However, there might be other reasons for the symptoms that your mother has been experiencing and a thorough evaluation by a physician with expertise in geriatric medicine could be helpful.

Need clarification: Don't know what HB always 76 means? Would be helpful to know if your physician diagnosed what type of irregular heart beat you were having? Two common types of "skipped beats" would be premature atrial contractions(PAC's) and premature ventricular contractions(PVC's).

PAC's are considered harmless. PVC's can be present in normal patients, but sometimes are a sign of underlying heart disease.

my mom 88 on dializes for a month now because of ckd and she also has heart failure she is doing very well no symptoms her blood pressure is around 146/51 but heart beat is in the mid 40's is that to low ? in the past beat was in the mid 50's ?

: Her mother should get an EKG to determine what rhythm her heart is in. If she has no symptoms, and the rhythm is a normal sinus rhythm, then no treatment is needed. If the rhythm is not a normal sinus rhythm, she may need tests to determine the etiology of the abnormal rhythm-like blood work, especially since she is on dialysis. 
So, call her doctor to get the EKG done. Review her meds as well!

Ekg: You are young and the voltage can appear to be elevated by ekg criteria . Need to adjust for age . More accurately an echocardiogram will be able to assess lvh.

Blood pressure: Need to to find out the cause first; anxitey, hyperthyroidism, cardiac arrythmia can cause palpitation , hypertnesion can be secondary to the above  or unrelated , see a doctor evaluation and appropriate treatment.

Heart rate: Heart rate can be very variable patient to patient an athlete can have a resting hr of 30-40 and that would be normal . It really depends if you are dizzy of lightheaded . Below 40 in most ppl is a bit too low.
