Weight loss: You will not lose weight without changing your diet habits.  Exercise is wonderful for numerous reasons but it will not help you lose weight unless you change your diet habits.  There is a saying that you can't out exercise a poor diet.  I would suggest a journal to keep track of your diet.  Consulting with a nutritionist would help you greatly.

Diet and exercise: You need to eat fewer calories and burn more. Decreasing starches like bread, rice, pasta, potatoes, pretzels, bagels, cereal often works well, but  any diet must be combined with daily exercise, like walking, swimming, bike riding, dancing.....

STOP JUNK fOOD: 12 yr olds have neither cars nor money, someone is buying it for her, stop it, what is to stop her from gaining another 50 kg in the next years? You would not let her stay out to 
1 am or go on internet unsupervised, so it is 
with food. Take her shopping and find out what low calorie substitutes she would like. List of worst food in order: drinks with
sugar, fast foods, fried foods, bad sacks.

Eat more: Well, first check w/your family doc to be sure there's nothing medically wrong to explain your underweight (http://www.Nhlbi.Nih.Gov/guidelines/obesity/bmi/bmicalc.Htm) such as infection or overactive thyroid. After that, it comes down to eating enough. Make sure it's not just high calorie junk food but actually larger portions of healthy food. Consider mediterranean diet (backed by evidence).

Nothing  specific: Keep active & a healthy diet but don't expect to grow taller.At 18 most males growth centers have closed making increased height unlikely.If you want to know for sure an x-ray of your wrist & hand can be analysed for bone maturity & tell if the growth centers remain open to new growth.

1100 ml/day fluids: is considered maintenance fluid requirement for someone 12 kg when maintained on fluids only, although the body can tolerate morebut excessive may cause diarrhea.  Many of the foods that one eats contain significant amounts of water, especially fruits,  veggies and milk,  so as the body metabolizes food, it provides essential fluid as well. If urine is light yellow, fluid intake is adequate.

Without more : Information i always recommend a patient with chest pain to be check by a physician. A good history and physical needed for a better assessment and recommendation. At your age i doubt heart condition, but better check to be sure.
