Bone spurs: Not necessarily.   I recommend you have an evaluation with a sports medicine doctor or rehab doctor first.    I doubt surgery is the answer.

i have heel pain on the right side of my left heel. hurts even when resting. it's on the bone so doesn't seem like plantar fasciitis. what could it be?

Varies: There are a number of other possibilities, the more common of which include tarsal tunnel syndrome, a calcanesl stress fracture,  a tendinitis of the Achilles' tendon to name a few.  Don't hesitate to be seen for an accurate diagnosis.

Inside or outside..: Which ankle bone has the small lump positioned under it. It is either on the medial (great toe side) or the lateral ( little toe side). It probably has little or nothing to do with the plantar fasciitis. There is no need to get too aggressive with the lump unless the symptoms increase in intensity. Consult with a foot/ ankle  specialist for the possibilities.

Heel pain: There are a number of reasons for heel pain. Most common being plantar facsiitis. If it is the calcaneous (heel bone) it may be a stress fracture or a bone bruise. There are some arthritic conditions that can cause this problem. Best to have it checked out by your local podiatrist or orthopedic surgeon.

Tendinitis : Or possibly muscle strain. Either way should heal with rest supportive shoes and anti inflammatory. Best to have it checked out to confirm.

Bone abnormalities: Your plantar fascia is the bow string of your foot which helps maintain the arch.  The fascia connect to the inner aspect of your heal .  It is common for this to get pulled and even get avulsed a bit and cause pain. Calcaneal spur is common. Osteochondral dissicans is when a piece of the cartilage that lines the joint breaks off. Looks like a puzzle piece was lost.

None: Most sandals lack proper support and cushioning. If you insist on a sandal you need one with at least a 1/2 inch heel.

Last resort: Normally this is a last resort since this is caused by improper biomechanical support. Injections, taping, orthotics and stretching exercises are often successful. Surgery to remove spur and release tension on plantar fascia can be considered if other measures do not work. Diagnosis should be re-evaluated first. Surgery if often but not always successful.
