Time plus: Time is the best healer but activity restriction, guided exercise, medication especially a steroid pack, and even epidural steroid injections may help.  Cessation of smoking, weight reduction and regular exercise will help prevent a recurrence which has a rate of 5% if due to a disc herniation. 90% are treated without surgery with 10% going to surgery with a 90% success rate if not a smoker.

Impossible to say: First of all, a significant disc herniation may not improve with conservative therapy. If it does respond, it depends on the severity of the nerve compression. Give it 1-3 months.

i have a l4-5 herniated disk with pinched nerve. pain is ok with gabapentin. but i do not want to take medicine for ever. should i go for surgery?

Accepted Answer

Nerve pain.: Gabapentin helps nerve pain. You may not need to take it forever even without surgery. Your pain may remit after a period of treatment with Gabapentin. This is not the reason for surgery.

See below: Your question is confusing.  Generally it takes a series of epidurals to get maximal relief.  If that fails there may be other possible injections or treatments for your pain.  There is always surgery of injections or physical therapy do NT help.

Lumbar epidural: Reduction of pain in the back as well as the lower extremities is a good indication that the epidurals have started to do their job. Numbness, tingling sensation, pins and needles and weakness in the extremities may not subside if there is nerve damage. But it is hoped the pain in the back and legs will improve. Often need more than 1 or a different type of injection to obtain the desired effect.

Yes: Sciatic pain can be due to an inflamatory reaction as well as direct nerve root compression or both.  When a disc herniates, there is an inflammatory component that can lead to pain without any nerve root compression still causing pain over the distribution or route of that nerve

Absolutely: In fact most sciatica is not caused from frank nerve compression but mostly chemical irritation from the disc problem affecting the nerve.

Sciatica= nerve com-: Pression of the nerves making the sciatic nerve. If no compression then sciatica is unlikely, except if the compression is by spurs of bone from arthritis or thickened l flavum or some other lesion after nerve has exited out of the spine.

can you have a pinched nerve without having any herniated disks in back but mild spondylitic change in l4-l5 (some facet degenerative change)?

Accepted Answer

Yes very much so!: The disk is the well known cause but arthritic spurs push the nerve against the bone, or thickened ligaments or against a bulging disk (not necessarily a herniated one).
If pain meds and therapy does not help, then Epidural or Facet joint injections can be tried. If nothing helps then surgical decompression may be needed.
See your PCP to refer you onwards to an Orthopod or a spine Surgeon.

can a gastric sleeve directly help with herniated disks / pinched nerve and knee problems (meniscus)?

Accepted Answer

Not directly: No, it will not help directly, but if with diet and exercise it allows you to lose weight, so of the symptoms associated with those other conditions may improve or even go away. Excess weight aggravates many conditions, including the ones you mention.
