You misunderstand : Ibuprofen/ Codeine 200/12.8 is not a fraction. It signifies a dose of 200 milligrams of ibuprofen together with 12.8 milligrams of codeine. A normal adult dose

If they were prescribed for you, take them as directed. If you found someone else's pills, get rid of them. Never take prescription pills belonging to someone else as they might do more harm than good.

Depends: It depends on how you metabolize it. Also why are you taking it?  Should  have a good reason to be on an opiod. Long term no matter how often you take it.

Gall bladder: Right upper quadrant pain and fever could signal gall bladder (GB) inflammation or infection, and narcotics like codeine can precipitate GB problems. Pneumonia also  can cause upper abdominal pain and fever. Please see a doctor for examination and appropriate tests.

does codeine expire? my 4 year old son has been dealing with a scratchy throat all day and the mucinex (guaifenesin) did not work today. we have some codeine that my older son used two years ago. is it okay to administer a small dosage to our 4 year old?

You : You should not give your 4 year old son Codeine for two reasons:

- first of all, Codeine does have an expiration date.  It the medication was given to you two years ago, then it is almost certainly expired.  Almost all medications have an expiration date.  You should go through your medicine cabinet every three months and discard any old, unused portions of prescription medications.

- second, it is never safe to share prescription medications unless specifically instructed to do so by your doctor.  Codeine can be a good pain killer, but can be very dangerous in the wrong doses.  Too large of a dose can cause someone to stop breathing.  That why many medications are regulated via the prescription system - for safety -  and should not be used unless specifically instructed to do so by your doctor.

The best (and safest) treatments for a sore throat are: lots of fluids, rest, hot tea with honey and lemon, chicken noodle soup, Ibuprofen or Acetaminophen for pain, and a spoonful of honey for a sore throat. Research shows that these home remedies are as effective and much safer than over the counter medications.  If you are concerned about your son's illness, you should call his doctor to discuss the safest and most appropriate treatments.

Six hours: I suggest six hours because the half-life of Codeine is somewhere between one and three hours. That means that half of your dose of Codeine is metabolized in somewhere between one and three hours and half of the residual is metabolized in another three hours. Usually in the young healthy person i would expect the metabolism to be a lot quicker than any older person.
So by six hours in a 14-year-ol.

Would need more info: Without knowing her weight, why you are giving it, and what form the codeine is in, it would be dangerous to speculate. For kids, if you are using codeine with acetaminophen liquid (Tylenol codeine elixer) you can generally safely give 1 teaspoon for every 22 lbs of body weight every 8 hrs up to a max of 4 teaspoons if they weigh 88lbs or more. Start with half dose to see how sleepy they get.
