No.: No, there is no evidence that any type of bra causes “sagging”. On the contrary, it makes sense that the use of a supportive bras may help prevent the effects of time and gravity.  Of course, significant weight fluctuation and/or pregnancies may also cause breast ptosis (sagging). Genetics may play a role as well.  Hope this helps.

Awesome question: I am not sure that there is an answer to this question. Intuitively, a person with large breasts would seem to sag more without the use of long term support but I am not sure that this has ever been studied because it would require a long term study with a bra on one breast and not on the other which is conceptually a hard study to perform. I do recommend that women with large breasts wear bras.

Size and Gravity: The best thing you can do is wear a bra, exercise and enjoy your life.  Gravity controls all of us here in earth, as well as our bodies.  Unless you are interested in breast augmentation, your breasts will "sag" over time, lesser degrees for some, greater degrees for others.  It will take some time though, years not months.

Its natural: Breast tissue is composed of connective tissue, glandular tissue and fat.  As we all get older, the less connective tissue we have so this causes the tissues to become less supported, to sag.

Gynecomastia : This depends if there is excess fat/breast tissue with or without loose skin. Usually this is treated with liposuction, and may require skin excision if not completely resolved with liposuction alone. If there is a tremendous amount of loose skin this may be addressed at the first surgery.

Lift vs Augmentation: If your volume is adequate than you may need a lift. If you require more volume that an augmentation my do the trick. Some patients require both to make the breast more youthful. Lifts are done by removing skin this will result in a scar.  You can see typical results on line at asps.Org.

Difficult: Giving ptotic breasts a more youthful appearance without a bra is difficult without surgery.  Surgical results can be very good with mastopexy +/- augmentation.  Mastopexy alone can improve shape but the breast will appear slightly smaller and significant upper pole fullness is difficult to achieve without augmentation in addition. Depends on the cause: Are they just sagging or are they sagging because of too much fat? If there is too much fat, a combination of ultrasonic assisted liposuction and direct removal of breast tissue can improve the contour and, through skin contraction, improve the sagging with minimal scarring. If there is just too much skin, you may have to trade sagging breasts for significant scarring. See a plastic surgeon.

Minimal help: Sagging breasts is a common problem.  Toning up the chest muscles (known as the pectoralis) will give some help but no enough of a difference.  The only way to get lift sagging breasts is the get a breast lift or mastopexy by a plastic surgeon.
