i have a pedunculated right fundal subserosal fibroid 5.6x 4.9 x 3.7 cm. is this considered a nice size? an what are my options for removal?

Laparoscopy: Just because you have a fibroid does not mean that you need surgery.  If it causes symptoms like pain, heavy bleeding, or infertility you may consider removal.  If your provider is skilled in laparoscopy this could be as easy as an outpatient surgery for removal.  If you decide not to have surgery then you should be followed on a regular basis.

Position: It describes the position of the fibroid (a non-cancerous tumor).  It is on the outside of the uterus, hanging off the uterus by a "cord" or stalk.  Depending on the thickness of the stalk, you could have fibroid embolization, and/or hysterectomy is still an option.

On a stalk : Pedunculated simply means that the fibroid is on a stalk hanging off the uterus. They are much easier to remove if necessary.

Fibroids: A fibroid which is posterior on the fundus and on a stalk ("pedunculated") may rest on the sacral area of the spine and cause some discomfort ( as back pain).  Laparoscopic surgery can remove the fibroid easily if this is a problem.  The simple existence of a fibroid of this size , without significant symptoms is unnecessary for surgery.

can a "us transvaginal test" identify if an adnexal mass may represent a pedunculated fibroid or other adnexal mass? if not, which test would? there is also a trace amount of nonspecific free pelvic fluid. thanks?

: An ultrasound procedure to define an adnexal mass should determine the difference between a "pedunculated" fibroid near the ovary coming from the uterus, and an ovarian mass which may require biopsy. "Trace fluid" in the lower abdomen is not unusual. See your GYN physician for guidance on the findings of the ultrasound.

what could eventually become of a pedunculated solid fibroid mass(golf ball size) if left alone? it is near uterus and pelv. side wall, not ovarian.

See below: It could bleed and you could become anemic. It could grow and cause pelvic pain.

Probably not : Fibroids may make your pregnancy more uncomfortable but are not likely to pose a risk to the baby. Your doctor will monitor the fibroid and the baby throughout the pregnancy

Are you symptomatic?: Sounds like you had pelvic us and reported the location of certain fibroids in the uterus. 3 locations: submucosal (central, against the uterine lining), intramural (in the muscular middle), and subserosal (outer, on the edge). Anterior (front) posterior(back). Fundal is top of the uterus. Pedunculated means it's on a stalk (but this term is often used to describe any fibroid away from uterus).

Not necessarily.: Many, many women have fibroids as they get older.  Most don't even know they have them.  If you are having symptoms due to them, like heavy periods, etc., then they may be worth treating. Discuss your options with your doctor. Very very rarely, a fibroid can become cancerous.
