Sounds normal: These velocities sound normal.  As your carotids narrow, the velocity of blood increases.  Sort of like when you put your thumb over the end of a hose.  The water moves faster and goes farther.

In many labs, the cutoff is 125cm/s.  Below this, there may be some narrowing, stenosis, but it is not significant.

Peak velocity: Sometimes what is being recorded is not the velocity in the internal carotid but an adjacent artery such as external carotid .  This could also be related to high flow situations or other technical factors.

How high is high?: Increased flow without stenosis can happen if the cardiac output is very high or there is tortuosity if the arteries Generally don't worry until systolic velocity is over 200 cm/sec and even then it is probably ok with normal diastolic velocity why did you get the test? IF you are asymptomtic I would not worry too much

i am 54, previous heart attack.  now have mild blockage in the right carotid artery and moderate/ severe in the left so i am told.  cta shows 50% blockage on both sides.  the left carotid artery has a velocity in the 400's. how should they treat this?

Stenosis/Narrowing: Presuming you're discussing an ultrasound finding, an elevated velocity (systolic or diastolic) can suggest the presence of a stenosis ("narrowing") of the carotid artery. Whether or not this is significant depends on how elevated and whether or not there are symptoms your doctor is trying to attribute to such a finding. I would discuss with a board certified vascular surgeon.

Probably: The common carotid artery divides into internal and external branches.  The internal carotid is much more important as it is the major blood supply to the brain.  When atherosclerosis develops it often starts at the branch point (the "bifurcation.")

Yes. : Any artery that is not "hardened" with plaque or calcium can go into spasm.  However, connecting spasm with clinical symptoms is a more difficult.  If you are concerned you should consult a vascular surgeon or neurologist.

Accepted Answer

Not really : Statins stabilize plaque so they are the best defense against atherosclerotic plaque. They decrease ischemic and embolic events. However, they do not reverse a stenosis.
