Straight leg raise: In general, you want to strengthen the quadriceps with as little pressure on the patellafemoral joint and tibial tubercle as possible.  forces in that area are maximized when knee is flexed.  A physical therapist could take you through the complete program, but straight leg raises is one of the major components of it; strengthening the quads while minimizing force across tibial tubercle.

Loss of cartilage: Chondromalacia means loss and/or damage to the cartilage that covers the bone. In your case, you have damage on the back and inside of the kneecap, and grade IV is the most severe grade of injury/loss. Patella alta means the kneecap is sitting higher (closer to the hip) than it normally should. We treat patients, not necessarily mris.  How you are treated depends on your symptoms and disability.

Ortho evaluation: If you have done the quad strengthening and other strengthening exercises and teen still having problems, it may be time to consider surgical intervention to relieve symptoms. I'd suggest at least a consultation with a pediatric orthopedist who specializes in sports injuries, even if this is  not happening associated with sports.

Stretch of ligaments: Sprains occur when ligaments (tissues holding joints together) stretch.  The ACL is a ligament in the inside of the knee.  The MCL  is on the middle side of the knee.  Disruption os ACL means it was probably/partially  torn.  Patella alta means that your knee cap is higher than usual.  You should  discuss these  findings with the ordering physician so she/he can determine appropriate treatmen

: You have to ask your doctor what primary problem causes your knee symptoms then you can decide what procedure you should go with. Fat pad trimming is for hoffa's syndrome and MPFL repair is for lateral patela tilt. I ain't sure if you truly have 2 conditions at the same time. If your knee problem is due to an accident or a trauma, lateral patela tilt is more likely the primary etiology. Good luck.
