My take is ...: What you've felt in L-testis and perineum is most likely a referring ache resulting from some disorders of muscle, joints or nerve fibers in the region, which could not be cured but modified with understanding and acceptance + behavioral modification + quality healthy lifestyle + pelvic physical therapy + judicious use of pain killers as needed + trying alpha-blockers, etc. More? Ask Doc timely.

No related to pap: Hi, a pap smear is a screen for abnormal cells on the cervix, not related to external  bumps.  If someone has  painful bumps it might be best if your health care provider sees them.  Painful indicates to me they could be infected:  either viral (possibly herpes) or bacterial.  It depends how long you've had them and if they come and go, etc.  They could be contagious, so it's a good idea to check.

Pimple: could be a pimple or hair follicle infection.  You really didn't ask a question here, but acne like lesions and hair follicle infections are very common in this region.  IF it appears to be spreading though, don't make the assumption that its a pimple.  Spreading pustules can be  the appearance of herpes as well.

morning after rough/not lubricated sex, noticed a small red cut on perineum. was painful. healed within 36 hours. is it possible it was herpes?

Don't irritate area: The simplest thing you could do is to not irritate the area any further.  That means don't scratch the perineum and wear loose fitting undergarments like cotton underwear.  This will require a little bit of will power but you will thank yourself later.  You could also cut your finger nails just in case to prevent anymore irritation.  
If this does not work than you could seek further medical help.

Needs evaluation: Lumps, bumps, blisters and other skin abnormalities on the genitals need to be evaluated. An examination will be needed to know the cause and treatment.

Painful vein: Small blood clots in superficial blood vessels can cause pain when touched. They are not dangerous, and the clot will usually dissolve in a few days at most.  Hot baths are helpful, and can stimulate local circulation which can speed the resolution of the clot.

my perineum is a bit swollen, redder than usual and painful to touch. i've never had any sexual partners or anything like that, what could it be and w?

Possible abscess: It could possibly be an abscess. I would definitely go have a checked out to make sure that it's nothing serious and also to have it treated before it could  potentially get worse.
