does the far pad wear away after a few years? i'm a avid walker but now i have pain under my big toe after walking.   what is the "cure" ?

See below: The fat pad can atrophy over time. Your pain can be from injury or from faulty biomechanics/not enough support. Try more supportive shoes and inserts. If that is not helpful, follow up with Podiatrist.

Pressure: too much pressure for some reason / tight heel cord contributes to this / if left alone it could result in an ulcer leading to infection and worse problems   this is worse for diabetics/  please get this checked soon with family doc or orthopedic surgeon or a podiatrist   sooner is better than later

Please get examined: Without seeing this, it could be as simple as a wart or callous. It also could be a lesion from a interphanlangeal sesamoid.
Please seek out a podiatrist and let him exam  you, he may need xrays.

See a foot doctor: This sounds like it is infected, injured, or gout.  You may need an antibiotic and hence should see a doctor, xrays may help rule out a fracture but with no history of trauma doubt that would be the case unless your a sleepwalker or was out drinking the nit before,
