My left big toe is 0.5-1cm skinnier than my right big toe, in the fat pad(?) area. it's been stable for 3 months, but i'm concerned it's atrophy. i have no other symptoms. i also passed a neuro exam, but she didn't look at my feet. concerning?

No, not concerning at all. Such anatomic variations are entirely normal. Don't worry about it.
