Possible, but: Having no pain or very little pain is a positive thing post surgery and can last for a long time, but other areas close to the initial site of surgery may be affected, or the nerves , muscles, ligaments, tendons or vertebrae in the area of surgery may have been strained by particular movement or event. Would recommend she call the Surgeon for follow up as she would require  physical exam /MRI even

Doctor can evaluate: Lower back pain can be due to many things. For young, active people, it is more likely due to strains and sprains. For older folks, there can be vertebral fractures, herniated discs, spinal stenosis and other compressions or irritations of the nerves coming out of the spine. Problems in the back part of the abdomen can also cause back pain, such as kidney disease, some cancers, etc...

Don'tRestYourTired--: Pain like that could be from the nerves constantly firing because their thresholds have been changed due to chronic pain or physical stress (it's called "neuropathic" pain), so they respond to lower levels of stimulus. There could also be inflammation in the area. Often, this kind of pain goes away with mild to moderate activity, then returns with rest. See your doctor for a complete exam.

Keep a movin: Of course the most common thing it could be would be just your usual low back pain.  You should never rest in bed for more than the first 24 hours.  After that, you walk, work and live through the pain being as active as you can with the help of ibuprofen or similar med.  If it doesn't go away, you should probably see a doctor to consider other possibilities.

Don't put this off: I had back surgery in feb, 2011 because i had a herniated disk (l3-l4.)  i was told if i would have waited longer, there could have been a lot more complications or delay in healing.  Don't hesitate to see a doctor.  An MRI may be needed.

Local heat: A heating pad, rest and taking ibuprofen may help to relieve muscle strain. If she feels weak, has tenderness  of the spine, an imaging study of the back may be ordered by your physician.

You : You should be seen soon by your regular doctor or consider going to the emergency room.  The fact that you are worsening, not improving, with time is concerning.  In particular, the onset of what we call "radicular symptoms" (i.e. Down your leg), is worrisome and requires evaluation.  You don't mention your age, medications or other medical problems, but particularly if you are elderly, suffer from other major medical problems or on medications such as "blood thinners" you need to get seen by a doctor soon. Doctor can examine: The more common causes of lower back pain include:  back muscle strain, back ligament sprain, fractured vertebra (spondylolysis), slipped vertebra (spondylolisthesis), herniated disc, spinal stenosis, spinal arthritis, urine infection, kidney infection or stone; tumor in the spine, kidney, or elsewhere;  fibromyalgia, etc...  With such a long list of possibilities, one's doctor can examine.

Doc eval needed: Any new change that you cannot explain how it happened needs to be evaluated by a doctor in person.  There is no way to explain what is causing this without a true history and md physical.  Good luck!
