Trauma that lead to: pain. Must be considered. Thumb ligament injury at the MCP joint, wrist fracture, in the radius or scaphoid are possible, as are other bone or ligament injury. tendon pain giving pain with thumb extension near the wrist can be a sign of contusion or tendonitis. An evaluation that includes xrays is typically a good step to take for the symptoms and history described here

Cervical disk?: Pain that extends up or down the arm might be due to a cervical disk with compression.  The history of falls is also consistent with this.  I would get an MRI and start with conservative treatment. Physiotherapy and nsaids work well and often alleviate the discomfort.  If ther is any weakness in the arm a consultation with a spine surgeon is  recommended.

Carpal tunnel: Not unique for a diabetic to have some nerve issues.  Perhaps your median nerve is compressed over the wrist causing carpal tunnel syndrome.  This should be readily reversible.  Talk to your doctor and perhaps get an EMG test to evaluated the severity and decide appropriate treatment.

Suggest Xray: Especially if you've had any trauma.  There are several things that can cause something like this, but if you've had an injury, you would want to make sure that there isn't a certain kind of injury (skier's thumb with stener lesion) at the base of the thumb that if not fixed properly can result in long term problems.
