If you know it isn't your heart for absolutely sure, other things can cause the patterns of pain you describe - radiating pain from your stomach or pancreas, bad gas, or a muscle strain are a few of the possibilities.

Muscle issues..: It sounds like you might have some muscle and possible nerve irritation in that area. If you suffered an injury, pushed,pulled, did heavy lifting this is most likely the consequence. Sometimes just by sleeping in the wrong position you may have this issue. Icing and anti-inflammatory meds can help. If persists or worse make sure you get checked ASAP esp. if not better or chest pain as well.

Could be: Pneumothorax or pneumomediastinum.  It is best for him to see a doctor for specific recommendation, because it can lead to breathing problems, which are true medical emergencies.

Maybe both: If the pain is different than what you have experienced before, you may need to get it checked out to make sure it is not something unrelated. If it is a flare of the previous symptoms it may be a combination of stress and/or pt. A good pt will work with you and adjust the program so that it doesn't make your symptoms worse.
