Get checked: Hard to know for sure without more background information, but description consistent with irritation of stomach or intestinal lining or possibly problem with gall bladder. If this persists, he should be examined by a physician and a few screening blood tests can be done to begin to identify the problem. Make sure he is not taking aspirin, ibuprofen or other nsaid medications.

No.: Fatty liver is a condition that is not painful. However, pain under your right ribs could indicate another problem. Gallbladder disease comes to mind first. Yes: If it is large enough and located in a site that would ordinary produce the pain.  It can stretch the liver capsule which is quite sensitive.  Gallstone disease is a more common cause of pain here and you'll probably want this ruled out; it tends to be colicky and less related to position.

The : The ct will be able to detect cancer if present. To answer your question, with any enlargement of organs there is a chance of cancer. However, there are also many other causes as well (fatty liver, liver cirrhosis, hepatitis, sickle cell disease etc.) hopefully your doctors will be able to figure out exactly the cause of your hepatosplenomegaly (enlarged liver and spleen) and come up with a treatment plan.

Probably not: The liver is located under the right side of your rib cage. Usually this doesn't cause pain, however sometimes inflammation of the liver causing the capsule to stretch in certain cases has been associated with pain. Without knowing the details, i would recommend visiting your pcp or a gastroenterologist for a workup with labs/imaging as it could be due to your gallbladder as well (i.e. Gallstones).
