Neuralgia: Pain radiating from under the shoulder blade around to under the breast may be due to compression(pinch) of a thoracic nerve as it travels along the under side of the rib.  Sleeping in an awkward position is one cause. The pain can be severe. It is usually a temporary problem but can last for several weeks. If OTC NSAIDS don't help gabapentin can be very effective.

Possible: costochondritis, or inflammation of the muscles and tendons in the rib cage.  Usually improves spontaneously. If not resolved in 1-2 days, please schedule an appointment with your provider.

Acs vs PE vs bronch: Would see an urgent care as it could be a sign you have a pulmonary emboli or an acute cardiac event or may be related to bronchitis or muscular skeletal pain.

Symptoms: Sounds like you may be an air swallower and have gaseous distension. You could try 'Gas-X' tabs, 2 QID for a few days and see if it helps. If so you need to learn how to swallow less air.

Costochondritis: Your pain is likely due to inflammation of the rib cage. If it is worse with  deep on deep inhalation, this is likely.  You can try an antiinflammatory like Motrin or naprosyn (naproxen).  It takes a few days to improve.   If the pain is exertional or  goes down the arm may be a heart issue   see your internist if no change in a few days. Good luck!

Needs to be watched: In your age group, the most common causes are musculoskeletal problems that usually self-resolve. Esophageal spasm can cause deep middle pain. Acid reflux is also a possibility despite taking tums (calcium carbonate). Young women who are smokers and are taking oral contraceptive pills are at risk for lung blood clot that can cause chest pain. Heart issues need always to be ruled out but are unlikely at your age.

: A pulmonary embolism is Blood clot, (usually originating from the veins of the legs), it breaks off, travels past the right side of the heart and lodges within the pulmonary arteries. Symptoms include severe shortness of breath (even death), increased heart rate and respiratory rate, chest pain and others.  Its possible the clot did not resolve, you had another, or maybe related to heart itself.

Multiple causes: Upper back & right sides chest may be the result of a thoracic disc herniation, thoracic facet syndrome, T4 syndrome, costoveterbral syndrome, muscle strain, ect. It may also represent referred pain from internal organs such as the heart & lungs, which are more serious. With your breathing difficulties you should contact  your  pcp for an immediate evaluation. If not available go to nearest er!

Chest pain: chest pain such as you describe is worth seeing a doctor for evaluation. It could be musculoskekeletal, but other possible causes also.
