These : These can be difficult problems to fix and often poorly respond to treatment including surgery.  Patients that have failed conservative treatment you may consider the following options:

1. L5-s1 disk with annular tissue - recommend discogram to determine if this is the cause of the pain followed by either endoscopic discectomy or fusion surgery.  Please see my health guides on endoscopic disk surgery.
2. Si joint pain - consideration for si joint fusion.
3.Coccyx pain - if pain is related to prior fracture sometimes surgical removal of the coccyx may give pain relief

i've coccyx pain since >2yrs; xray showed nothing; mri showed disc hernia l5/s1 and a nerve in that area being pinched. could there be a connection between the herniated disc and the coccyx pain (physiotherapy, medication and shots didnt help?

The Ls-S1 compre -: -compression @ L5-S1 would cause pain radiating down the posterior leg, to the lateral aspect of the ankle &amp; foot. If severe, weakness in the gastrocnemius-solious (sp) complex, meaning unable to stand on your toes on that side.

Herniated disc: Not too much to get it back into place - given that it is a large one. But you do things to make it less symptomatic - PT, core strengthening, injections, medication, Stem Cell/PRP disc injections.

Accepted Answer

Depends on pain: Most disc herniations heal with time.  Depending on pain levels, medications like antiinflammatories and muscle relaxers are often helpful.  If there is severe enough pain, an epidural injection may help.  About 10% to 15% of patients need surgery.

IT SHOULD: The large nerve in your back gets smaller and smaller until it terminates in the toes. Unless permanently damaged, it should resolve.

Numb big toe: Sound like your L5/S1 disc is involved if you are getting numbness in your big toe. A neuropathic med like gabapentin, lyrica, (pregabalin) etc. may help with numbness. Your L5 nerve root is affected.  Might benefit from a selective nerve root block of your L5 nerve root with some steroid like dexamethasone, celestone, etc. Physical therapy may help your back symptoms too.

Can be intermittent: If a herniated disc causes signs like straight leg raising or sciatic nerve tenderness, it can be intermittent. So can any neurological or muscular sign or symptom. Left alone most resolve with conservative treatment. And some people with herniated disc on imaging studies have no pain.

No: However, it can cause muscular symptoms.

Accepted Answer

Not really: Although genetics may make someone more prone to having a herniated disc (posture, ligamentous laxity, muscle strength, etc) the actual disc herniation is usually from injury or trauma.
