fell snowboarding 3 days ago. pain in lower right clavicle and neck. hurts most when i sleep, cough, or certain movements. any ideas?

See below: Sounds like you may have whiplash.  This can occur with falls.  You likely strained your neck muscles, but it's also possible to injure the joints, discs, or nerves in the neck.  This will result in referred pain to the neck and/or shoulder.  Be sure to see a physician for an accurate diagnosis.

i am having dull collarbone pain, shoulder pain, and neck pain on the right side. hurts to cough as well.

Hard to say: For this type of complaint a face to face exam and history is required. Did you get into an injury? Are you having a respiratory infection? Is there chest pain? You'll get more answers with an exam by a doctor. I would see one at an urgent care as soon as you can.

4 months ago upper endoscopy positive for gerd, gastritis, and hiatal hernia. more recently i sometimes have right sided neck/shoulder/collarbone pain after (and infrequently during) eating. unlikely to be anything insidious or malignant?

: This can be referred pain from diaphragm irritation, which can happen with the hiatal hernia that was diagnosed during the endoscopy. If this continues, you should see your GI doctor to see what treatments are available for the hiatal hernia. Good luck!

woke up with collarbone pain on the left side. it bothers me every now and then but it goes away. its still lingering. i have a herniated disc in my cervical spine. it hurts to the touch. not sure if i slept wrong?

: A herniated cervical disc doesn’t usually cause collarbone pain, but rather numbness and tingling and sometimes pain going down your arm to your fingers. 
Are you sleeping on your side or on your stomach? Have you sustained a fall or injury? Any local redness or swelling?

have had 3 cortisone shots in last 6 months for bursitis in left shoulder. now i have  ripping pain in clavicle. related?

Maybe: 3 cortisone shots in the last 6 months in the shoulder are a lot.  You may need a x-ray and maybe to make sure that nothing new is going on your shoulder joints such as avascular necrosis.

Sternal clavicular: Sternal clavicular joint pain can come from a few different causes.  The most effective way to treat is first to obtain a diagnosis from an orthopedic surgeon.  Once the cause of your pain is diagnosed you can follow a treatment plan.

the base of my neck where the sternocliedomastoid connects to clavicle area hurts to the touch. what could be the cause?

Muscle pain: The most likely cause is a muscle strain. The sternocleidomastoid functions in neck rotation, so a strain can result from overuse of the neck muscles or abnormal neck positioning. Another cause could be infection of the overlying skin and or muscle itself. This would be associated with redness and warmth in the area. Treat initially with ice, anti-inflammatories, rest. See a doctor if not better

i have bad neck spasms, could this be due to a plate to reset my clavicle?

Possibly...: Best to let your doctor answer this question; this may be the cause, or your may suffer from degenerative cervical disc disease.  A visit to your primary care physician is appropriate.
