Night pain is always: A symptom that should be evaluated immediately as malignancies may present with pain worse at night.  Wih bladder control issues, there could be an issue within the pelvis region or spine.  You need to see a physician now or go to the er.  It could be a urinaryinfection as well but this needs immediate attention in any case.

Low Back Pain: It would appear that you have  lumbosacral disc pathology with some compression of the nerves that go down your legs. You will require a lumbosacral X Ray and MRI to determine the exact pathology. Since you have indicated that this is a chronic condition, it would be good to consider an epidural injection if indicated, to help relieve the symptoms of pain in your legs. No strenuous activity please

You should see a neurologist for evaluation-you will need to be examined and then testing done to determine the etiology of your symptoms. There is treatment depending on the exact diagnosis. One option: Diapers are one option for a person who lacks bladder or bowel control. But there ae other options as well, such as intermittent catheterization and intermittent bowel cleanse so the person doesn't soil themselves.  There is not a one size fits all (pardon the diaper pun) answer here. Each case has to be decided individually for its pros and cons to determine best course of action for that person.
