A few thoughts: You have to consider what is behind the knee. There are blood vessels, tendons, muscles, knee joint. You have to consider each of these. When you are evaluated these will be looked at to try to clarify your symptoms.

See below: Hard to say.  Get seen for a physical exam.  There may be anything from a muscle, tendon, ligament strain, baker's cyst, or other issues related to tissue around the knee.  But a DVT must always be considered and your doc, with an exam can help to figure if DVT testing is needed.

Baker's cyst?: Several causes of pain behind the knee. I assume no recent trauma--you may have a baker's cyst--basically a collection of joint fluid extruded outside of the joint and formed sack there behind the knee...As it enlarges, it can cause pain and reduces ability to bend the knee.
A bit clot in the back can do so as well, but you don't have other symptoms to suggest that.

Cartilage/ligament: This could possibly be a either a meniscal (cartilage) injury or a torn ligament.  Any time one describes a knee "giving out" it sounds like a possible ligament injury, although with a ligament injury most likely you would have known there was an injury.  A meniscal tear can be tricky and a possibility.  Any time knee pain more than 2-3 weeks despite rest and not improving consider ortho visit.

Posterior knee pain: Yes, see orthopedic surgeon. Consider arthritis, gastroc soleus strain, popliteal cyst, referred sciatic pain, and rule out a blood clot with venous doppler.

slipped on ice and fell yesterday. pain behind knee. no swelling or popping. fine when i rotate it or squat. but it hurts when i walk. what did i do?

Rest ice elevation : Does not sound like a ligament injury given your symptoms. Also unlikely a fracture. Ice the knee twenty minutes on twenty off, elevate when resting, take an NSAID such as alleve or advil (ibuprofen) as directed. ( of course do not take if you can't medically such as allergy, if your on blood thinners, or have severe gastritis). See an orthopedist if your symptoms are not improving in a week.

Possibly: Unfortunately, even the physical exam looking for  a DVT is only about 50 percent accurate.  So if you do have posterior knee pain and particularly swelling in the leg, you may need to be seen for an ultrasound.  You could very well not have a DVT, but having one is potentially life threatening.

Not DVT: The negative D-diner test effectively rules out deep vein thrombosis as diagnosis.

Your symptoms suggest orthopedic problem in knee or sciatica.

2 back surgeries in ~1 month. wondering if increased swelling@l5-s1 from overdoing it could cause severe burning/cramping pain behind knee, in calf/ankle, & thigh? have to lie down & hard to walk/sit.

Yes: Overdoing so soon after back surgery can cause nerve irritation which will result in symptoms the you descibed.  You have to take frequent break and lie down to get relief. Good Luck
