NSAID's & Tylenol (acetaminophen): Nsaid's stand for non-steroidal anti-inflammatory drugs which help reduce pain via decreasing concentration of inflammatory markers.  Tyelnol has been used as a pain killer for a very long time but seems to do a better job at reducing fever.  Otc these oral medications are your best options.  Topical drugs that are applied to skin such as biofreeze have been helpful as well.

TMJ pain: more information would certainly be needed to make a proper evaluation of the cause of the pain. I would suggest seeing your dentist as soon as possible to see what the possible causes might be

Need evaluation: Your symptoms are not consistent.  If you have lack of appetite, you should be eating less and losing weight, not gaining.  Your TSH level is normal though in the upper range of normal.  Although you could take a slightly higher dose of synthroid (thyroxine) to correct this, it will likely not change your symptoms.  The joint symptoms and pain should be evaluated by your doctor.

Lumbar spine pain: This could be lumbar spine spasms that could be due to acute lumbar muscle strain or if it persists, could be due to a disc herniation. If the back pain worsens (severity worsens or it becomes more disabling more days during the week) as time goes on or (more importantly) if the pain travels down the leg(s) then seek medical attention.

Plenty: Most narcotics and controlled substances make you nauseated and increase the chance or drug addiction. For a torn rotator cuff I'd recommend a shoulder immobilizer/sling so it doesn't move.  You can ice over that. If over the counters aren't covering the pain a  steroid pill or shot could be considered. You could see your doctor for other alternatives. Good luck

i have ms and trigem neuralgia and have terrible pain. tried gaba, baclofen, norco, (hydrocodone and acetaminophen) elavil, tegretol, otc, etc...what is the best pain med for this?

Trigem neuralgia: There is no best pain med. Short term certainly stronger opioids may be tried but this is not a long term solution. You need to see someone about getting some nerve blocks etc.. Ultimately if nothing works then you have to consider a microvascular decompression of the nerve.
