Approximately 1 year: Depending on how it was injured it may take up to 1 year for the nail to fully grow out.

Could be fungus in: the toenail.  A doctor or podiatrist can evaluate and tell for sure.  Sometimes the nail grows back thick because the nail bed has been damaged.  A culture can be taken of the debris below the nail, or but cutting a piece of the nail and sending to a lab.  fungus infections of the nail can be treated but the treatment is long and the nail grows slow so you may not see changes for months and months

Yes: As the new nail grows it wil push the old nail off.

They can: Due to their size, the larger toenails may grow slightly slower than the smaller toenails.  Overall the time frame will be close unless you have an infection in one of the toenails, as that would slow down the growth rate.  Good luck.

Nail diagnosis: The most common reason for this is either a fungal infection of the nail or a skin disease such as psoriasis.  

There are several treatments for both including topical therapy, oral medications and alternative treatments such as laser treatment for toenail fungus

Microtrauma: It sounds like your toenails are hitting the end of your shoe.  This usually happens if the toe box is either too small (toe is pressed up against the end) or too large (foot is sliding around and the toe is hitting the end).  This is very common in runners--it can lead to either thickening of the nail, or it may eventually turn black and fall off--a new nail will grow in within a year.

: Is the mail “normal” looking being clear ? Does the mail have a fungus infection? Could the shoe have rubbed the nail? More information is needed. Not usual for a toe mail to just fall off. No other unusual signs or symptoms ? Have examination by podiatrist or family doctor.

Time Line.: The nail plate is, in a way, a crude calendar  of events regarding the history of the toe. That line, depending on it's position on the toe, indicates that something happened to influence the nail root. This typically is in the form of an injury. Understand that the injury doesn't necessarily have to be a traumatic event full of pomp and circumstance. It could be as subtle as a tight shoe.
