Acute glaucoma-c doc: It sounds  like you need to see an eye doc asap. It is possible that you have acute glaucoma (increase pressure inside eye), or a significant infection which needs to be evaluated and treated urgently. Don't wait, please call doc now. Good luck.

Anything: Could be as simple as migraine or something more serious.  There are disorders called trigeminal autonomic cephalgias that are different from migraine and can lead to "red eye".  I suggest a work up from your doctors.

: This sounds like a pretty serious problem, maybe an internal eye infection (endophthalmitis), which could damage the eye and risk blindness. It isn't necessarily related to snorting heroin or anything else. He should return to a doctor immediately like ER or urgent care clinic. He may need consultation with an ophthalmologist, infectious diseases specialist, and/or neurologist. Don't delay!

Some thoughts: Couple common problems can do this: corneal abrasion (scratched eye)-perhaps something got into that eye. Subconjunctival hemorrhage-when a tiny blood vessel beneath the surface of the eye breaks (not always assoc with a known injury). It's usually painless and resolves w/in 2 wks w/o specific tx. If you have other bleeding/bruising problems, high bp, vision loss/changes, significant pain see doc.

Many things: If it actually looks like a small amt of blood, could be a subconjunctival hemorrhage, which is common, painless and doesn't affect vision.  Other causes of red eyes (of which there are many) can have an asymmetric presentation, anything from pink eye, blepharitis, dry eye, allergies, hsv conjunctivitis, etc.  Mild cases of these things may not cause pain.

Time for re-check: Three weeks from an initial ear infection is prime time for a repeat. Any infection creating a hole in the drum would likely seal over/heal initially but that too deserves a re-check. I would do so at your convenience. It may be something else that is troubling him.

See the doc: Without knowing more about the situation, i would recommend having a doctor check it.  It may be a common garden variety conjunctivitis, but it may be something else.  Other important signs are the presence or absence of pain and changes in vision.

Tx cause: Depends on the cause. If from "pink eye"-time & maybe antibiotic eye drops. If from allergies-tx w/ allergy med. If from subconjunctival hemorrhage (tiny broken blood vessel in 1 eye only)-heals on it's own w/in 2 wks. If from corneal abrasion or ulcer-need to see eye doc for tx. If from lack of sleep-sleep more. If not improving w/ initial tx-pls see your doc. Good luck!
