This sounds like a pretty serious problem, maybe an internal eye infection (endophthalmitis), which could damage the eye and risk blindness. It isn't necessarily related to snorting heroin or anything else. He should return to a doctor immediately like ER or urgent care clinic. He may need consultation with an ophthalmologist, infectious diseases specialist, and/or neurologist. Don't delay!

suggest you see an ophthalmologist for a proper diagnosis, this may not be an infection.
