I : I am very concerned about your symptoms.  I would like you to see a neurologist and have a complete evaluation of your symptoms.  Your symptoms sound somwhat like a concussion, but the duration is very concerning and you need a full neurological evaluation.  If you are having this severe of symptoms then you should not be participating in any type of sports until you are cleared.

In addition, you should see your ENT doctor again to re-evaluate your persistent injury.  But seeing the neurologist is most important at this time.

If you have worsening headache, visual changes, vomiting, weakness or numbness then you will have to be seen as an emergency.

Very dangerous: There's a syndrome called "second impact syndrome" which can occur as someone has a second concussion while recovering from their first concussion.  The resultant injury to the brain can cause malignant cerebral edema (swelling) and this can be fatal- mortality is 50-100%.

In most cases: The bleeding for most patients are from the front of the septum. Lean forward (so you won't swallow your own blood) and pinch the nostril with your isilateral thumb on the side that is bleeding for 10-15 minutes will usually stop the bleeding. Decongestant spray like afrin (oxymetazoline) can also help. If the bleeding continue and is severe, you'll need to go the emergency room for treatment.

See your doctor: I recommend you see your regular doctor, a sports medicine doctor, or neurologist.  If your symptoms are bad, worst headache of your life, slurred speech, double vision, nausea, vomiting, then go to the ER.  You need to be further evaluated.
