: Average iron saturation is 15 to. 55% so your iron saturation  and ferritin levels are normal.  No reason to act on these lab results.

Likely: The best test for iron deficiency anemia (ida) is ferritin. Sometimes when there is inflammation (an possible associated anemia of chronic disease/inflammation, the ferritin may increase as an acute phase reactant). The "gold standard" for diagnosis is a bone marrow biopsy, but rarely needed. High platelets are also associated with ida. Treatment should improve platelets and ferritin.

Suspect so: If the serum iron is in micrograms/dl, then this is not high but within the normal range, as it can be in iron deficiency in which the patient has perhaps taken an iron tablet or eaten steak recently.  If the units for TIBC are micrograms/dl, then TIBC is distinctly high and this speaks for iron deficiency in any case. Don't treat iron deficiency without seeking a cause.

is it safe to take otc iron supplements to raise (for one month, 27 or 65 mg), lower than normal ferritin levels?

i ordered this myself bc i have internal hemmhroids which bleed alot.

Okay and: Get your internal hemorrhoids treated. Continuing blood loss is not normal. While taking iron and multivitamins may forestall anemia, it is not advisable to leave the underlying cause untreated. Internal hemorrhoids are treatable through more than one modality. See your doctor.

: To have benefit from iron supplementation (FeSO4 ferrous sulfate) serum ferritin levels should be over 100.  With "heavy periods" continually depleting red cells, iron supplementation is certainly justified.  Ask your physician about monitoring serum ferritin levels, so that excess iron does not accumulate and cause other problems.

Fluctuate: Ferritin is an acute phase reactant and levels can vary considerably even in the absence of illness. The key is that very high or very low levels do assist greatly with diagnosis of iron-related illness, and it's useful in monitoring response to treatment for hemochromatosis.

Many possibilities: Your question is why your hemoglobin is at the top-of-normal (or maybe a tiny bit above) for a person of your age and gender. I'm much, much more concerned about your chronic osteomyelitis. I'd consider a right-to-left cardiac shunt, osler-weber-rendu lung shunt, sleep apnea, high-affinity hemoglobin,  an obscure mutation. It's not dangerous in itself & could "just be you.".

i've type 2 diabetes, controlled by diet (a1c level is 5.2), but i'm a healthy woman with regular diet/periods. my blood work came back with normal serum ferritin level (26 ng/ml) but high iron and iron sat (67), low uibc (120). please advise - thank?

: If you're not taking to much iron supplements and have a high serum iron with a low UIBC, hemochromatosis would be a consideration despite normal ferritin level which can occur early in that disease especially in women who lose iron during menses for years.. Do you have a family history of hemochromatosis which can also cause diabetes?. Consult a hematologist for evaluation and advice

U R iron deficient: Probably. You'll start feeling tired before you become anemic as the iron that your cells need for metabolism are deficient. You need to find out why you are iron deficient. If it's pregnancies, your monthly loss, or an ill-advised vegetarian lifestyle, correct it. If not, ask your physician how to identify possible blood loss.
