Hard to say: This may need surgery but it depends on your orthopedic surgeon as to what's the best course for you. Either way you'll need a cast.

i had intra-articular fracture of both heels over a year ago and now i have arthritis in both ankles. pain is agonizingly high and my question is, will the pain progress and get worse with time or is it going to get somewhat better?

Physical therapy....: This is rather difficult to predict, especially given your young age. But, you can go to physical therapy and learn different exercises to do that will hopefully prevent further progression of the arthritis over time. The physical therapists can also see if any type of splinting , or other modality, would be helpful to decrease your current pain. Good luck to you!

Yes: A weber b fracture is at to slightly above the level of the distal tibia and fibula joint, and should be considered extra articular.  Depending on the fracture pattern there may be a small propagation of the fragment below the level of the distal tibia.

i have a non displaced fracture in the middle phalanx of my index finger. the er mentioned intra articular extension. whats the usual treatment for that?

: Most phalanx fractures will heal without surgical intervention - (approximately 2% nonunion rate). With non displaced fracture of the middle phalanx of the index finger, it can be treated with dynamic splinting 2-3 weeks.

Wrist fracture pain: It is very unlikely that pulling or traction would cause comminution. Pain more likely from soft tissue stretching, including muscles, tendons and even nerves. If doesn't begin to improve or if you have numbness and tingling you should notify physician

"outside": It means the fracture does not enter or include the joint (which means it is much less complicated than it could be, and more likely to heal without significant arthritis / pain). Still needs orthopedic care, though. Good luck!

Related to the joint: Fracture of the femur  and other bones can be described as Extra/Outside of joint , Partial/involving part of joint, complete/entire joint, Intra/inside of the joint . Treatment can sometimes depend on the type of joint involvement.

Ankle Fx: There are multiple different ways the fracture could move.  If the cast is loose, contact the professional that applied the cast and make another appointment immediately.  As stated earlier multiple different fractures and conditions can arise with a loose cast.  This is an important time of healing for your body and a stable cast is prudent for this to occur correctly.
