Cystic acne: It sounds like you might have a secondary skin infection in addition to acne on your face, or that your acne has become cystic.  This usually requires oral antibiotics in addition to topical acne products, and I recommend that you see a dermatologist or your family physician, who can start you on Rx.

: (I used to practice in Cupertino.)  As long as ovaries remain, cycles without menstruation will continue.  I have no explanation for prolonged duration of "PMS" but understand why a gynecologist would preserve ovarian function at age  31.  I would advise discussion of your hysterectomy with the surgeon--many facts are unavailable to me

: PMS is generally hormone related, not a warning sign of cancer. Sometimes an ovarian cyst can cause a lot of those symptoms, and cysts are more common after hysterectomy. Maybe try to schedule an ultrasound with your gynecologist?

You're a man now!: Acne comes and goes for all young man and many older men. It's got nothing to do with what you do in private, or what else may happen as you develop further as a man. When it's on the back, benzoyl peroxide won't work too welli start with topical Clindamycin with or without a retinoic acid derivative, but often its presence on the back suggests one of the systemic antibiotics may be right for you.

Becoming a man: "Night fall" is a term often used by misguided people for the normal process of nocturnal emissions. They're healthy and you should enjoy them. You need to manage you acne with topical benzoyl peroxide as tolerated and prescription-strength Rx if not clear. Things sound like they are on course if perhaps later than your peers. Talk with your physician about any changes that concern you.

Water may help: Although proper hydration may help acne, washing your face with water may not be enough to remove all dirt or oils from your skin. You may need to invest on a nice cleanser for your skin type.
