i have had hemmroids and an anal fissure over a year now. they gave me some nifedipine lotion... is there hope ? do  fissures worsen during pregnancy?

Maybe: Hemorrhoids are expected to get worse during a pregnancy, but no necessarily a fissure, although anything can happen, consider a consultation with a colorectal surgeon. That doesn't mean you will have an operation, and you will get all the treaent options. Hope this helps!

Where it hurts: Apply it to the area that feels irritated.Application directly on the fissure is fine.

Nifedipine : It is a vasodilator so it gets more blood to that area so the Lidocaine can take quicker effect. It can also help to relax your sphincter tone.

Banding: Your pain is not from your hemorrhoids.  Banding in this instance is not a good idea.  You likely have an anal ulcer or fissure and if you do not respond to topical nitroglycerine (Rectiv) or nifedipine then you will probably need a 5 minute procedure called sphincterotomy for permanent relief.  You should also be using bulk (Konsyl, Metamucil, etc) and softerners (docusate) daily .

Maybe,maybe not : The dose you listed is considered more than adequate for the medication to work. Both 2 and 3 x daily dosing has been suggested in references. A variety of side effects including dizziness, nausea, light headedness, pallor and heart rate changes are occasionally seen and may be more likely with a larger dose.Larger doses will not necessarily speed healing.

Yes: Just because you have a fissure does not mean that there cannot be a polyp or tumor in the rectum or sigmoid colon.  Your doctor is doing the right thing by checking. Hope that helps!

Fissures hurt.: Anal fissures hurt really bad. They burn and take time to heal. Use the Nifedipine as directed by your doctor. They use it to relax the sphincter which when too tight causes skin breakdown.

Safe: Nifedipine is safe in lactation. Even safer is topical Diltiazem which is a first cousin of nifedipine. Most anal fissures heal in a couple of weeks. If you still have issues after 6 weeks, Botox or surgery is indicated. Meanwhile keep your stool really soft by eating at least 1 cup of fiber one or all bran cereal daily. Good luck. I know it hurts!

Cream(s): I usually recommend a nitroglycerin-based ointment.  Also, some pharmacies can make up their own concoction with the collaboration of your doctor. This is called a compounding pharmacy. Perhaps talk with your local compounding pharmacist to see if they have a formulation that is helpful. It is vitally important that you find out why you're having this problem and prevent it/let it heal. Good luck
