What is the source?: Sciatica is commonly caused by a disk protrusion in the lower back in a young person.  There are other less common causes such as piriformis syndrome (hip area).    While a lumbar epidural steroid injection in the lower back can help if a disk is pressing on the nerve there.  It is not going to help if the cause of the sciatica is somewhere else.  MRI and examination can help pin point the cause.

Sciatica: Depends on what is causing the sciatica. Sciatic nerve irritation can happen at the vertebral level and causes pinching of the nerve at the nerve root level. Of then this can need steroid to improve. Sciatic irritation can happen at the level of buttock with pyriformis or obturator spasm. This can be better manged with a muscle relaxer

If these treatments aren't helpful, surgery may be indicated and may give the quickest relief.

Yes.: Diclofenac is an anti-inflammatory medicine. It is indicated if you have failed the use of tylenol/acetominophen.  Be careful of the gastrointestinal side effects of any NSAID that is taken.
