i have occipital neuralgia and medications and nerve blocks have stopped working. i have read about surgical options. what would you recommend i do be?

Nerve pain: Occipital neuralgia may at times be secondary to nerve root irritation in the upper cervical spine. For cases that fail to respond to blocks, I have successfully used peripheral nerve stimulation. It is a form of implanted device and can work extremely well.

Talk to Neurologist: There is not enough information about trigeminal neuralgia and pregnancy because most people with trigeminal neuralgia are over 50 years old. See a neurologist as they can make all the difference.

PENS: Per-cutaneous electro nerve stimulation is available at many pain management sites. Electro acupuncture may also give relief.

Neuropathy: That is a difficult question to answer online. A good neurologist might be able to determine the true cause for your pains. Sometimes inherent nerve problems point to a generalized neurologic disorder. Sympathetic chain surgery might also be beneficial. Maybe a second opinion?  Best of luck!

Typical meds: Tegretol, neurontin, Lyrica (pregabalin) are standard first line treatment medications.

There are a number: Gabapentin and Trileptal are common. Clonazepam, Dilantin and Baclofen are less effective. Some a combination of these meds are used.

Accepted Answer

Migraine means: vascular, i.e. a painful pulse due to relaxation of smooth muscles in meningeal arteries walls, artery dilation driven by blood pressure (more so if elevated). The enlargement stretches the meningeal membranes around the arteries (which are very pain sensitive). Where is the control of all this? Within one's brain (mind), yet subconscious. Let me explain: It can be difficult to distinguish occipital neuralgia from other types of headaches — thus, diagnosis may be challenging. A thorough evaluation will include a medical history, physical examination and diagnostic tests. A doctor can document symptoms and determine the extent to which these symptoms affect a patient's daily living. If there are abnormal findings MRI and cat scan could help.

if carbamazepine isn't working to treat my trigeminal neuralgia, what is a good drug to try next?

Varies: Carbamazepine used to be the main agent in treatment of trigeminal neuralgia years ago.  We now have a multitude of anti-neuropathic agents with better side effect profile. To list a few, we have lyrica, (pregabalin) neurontin, topamax, etc.  The choice is based on coexisting medical issues and other medications on which patient is placed.  The physician can make this determination on a case by case basis.
